Job Title: Occupational Therapist (Band 6) Reporting To: Head of the Hospice Inpatient Unit Accountable to: Patient Services Director Responsible for: As part of the multi-disciplinary team providing Occupational Therapy support to patients and carers on the Hospice Inpatient Unit. Hours: Part-time 2 days per week (15 hours) - preferably Tuesday and Friday - Hours to suit/can be school time hours. Location: Heart of Kent Hospice, Preston Hall, Aylesford Salary: £41,354 FTE Pro rata £16,541.60 per annum This is a fixed term contract for up to one year to cover maternity leave. Participating as one of the multi-professional specialist palliative care team in the development and delivery of a co-ordinated and comprehensive palliative care service to patients, their families and carers and taking the lead in discharge planning for patients going home from the Inpatient Unit. An exciting opportunity has arisen for an Occupational Therapist who would be interested in expanding their palliative care skills on a Hospice Inpatient Unit. This is a fixed term contract for up to one year to cover maternity leave. Working with IPU multi-disciplinary team, supporting the patients to develop their independence and autonomy with activities of daily living, involving the patient, carer, family and MDT in the treatment plan. This may involve providing education on symptom management or equipment use. Essential Criteria for the Role - Team player - Good communication skills - Can do approach - Professional registration - Able to demonstrate up to date evidence-based practice About our Team Colleagues on the Inpatient Unit consist of a multi- disciplinary team which includes nurses, healthcare assistants, trainee nursing assistant, clinical administrators, doctors, physiotherapist, housekeeping and volunteers. This team works with the wider Hospice MDT to provide person-centered, holistic care and support for patients and their loved ones. About us 1.1 Work with the patient to develop their independence and autonomy with activities ofdaily living, involving the patient, carer, family and MDT in the treatment plan. This may involve providing education on symptom management or equipment use. 1.2 To provide adaptation/equipment as appropriate to aid daily living, and to provide instruction in their safe and proper use in the Hospice and at home where appropriate. Working as part of the IPU MDT (including sessional Physiotherapist), take a lead role in the planning for discharge from the Inpatient Unit, ensuring community follow through for patients with complex needs, particularly related to OT. Undertake home assessments as required to support IPU discharge. 1.3 Within the scope of your knowledge and skills, plan and deliver sessions as part of the Hospice Hub in Magnolia Place Outpatient Centre. 1.4 To be a specialist resource to other members of the multi-professional teamproviding up to date information on equipment to promote patient independence. 1.5 To communicate and work closely with all members of the multi-professionalteam, including complementary therapists and volunteers in the inpatient unitand Hospice Hub in Magnolia Place Outpatient Centre. 1.6 Be instrumental in establishing a link between the Inpatient Unit and Magnolia PlaceOutpatient Centre, ensuring inpatients are encouraged and supported to access the Hub programmes and activities during their inpatient stay. 1.7 Work closely with the MDT to promote rehabilitative models across all settings, including advice and support. 1.8 To be creative and implement new ideas in practice. 1.9 To ensure that practice is evidence based and to encourage the use of research in the further development of clinical practice. 1.10 To liaise with other statutory, independent and voluntary organisations regardingpatient care as appropriate.
