We support staff in clinical practice in many ways, and this includes having a team of Practice Educators to support and guide staff as they work alongside them. You will need excellent assessment and communication skills. In addition, you will carry a caseload and participate in the teams "shared care" rota. As a member of the team, you will need to work alongside other agencies to promote holistic recovery focused care as well as producing reports, completing audits and running clinics as necessary. We will provide training, shadowing opportunities and support you to work autonomously in the community and use your own initiative. The successful candidates will have sound clinical skills, be able to coordinate complex care packages, have good administration and IT skills, as well as a good working knowledge of CPA processes, report writing, organisational skills, safeguarding knowledge and the ability to identify issues and demonstrate ownership of change processes. *To act as care coordinator to an agreed caseload of service users with severe and enduring mental illness. *To undertake ongoing assessment, treatment and evaluation for service users known to the service. *To work in a flexible hours service, including providing services outside of office hours and at weekends and bank holidays.
