As a GP in Stalybridge, Dukinfield & Mossley PCN, you will provide patient consultations for both acute and routine cases across 9 practices, managing a varied caseload. You will care for patients with minor ailments, chronic diseases, and undifferentiated medical conditions while ensuring high-quality, protocol-driven care. You'll handle referrals by assessing patients and making onward referrals to specialists or secondary care, working collaboratively with the referral management team. For prescription management, you will review, sign off, and process prescriptions, supporting other PCN healthcare professionals with medication reviews, repeat requests, and resolving prescription-related queries. Collaboration is key, and you will work as part of a multi-disciplinary team including nurses, pharmacists, and ARRS professionals. You'll also contribute GP input and leadership to support patient care coordination across the network. In addition, you'll participate in initiatives that extend GP services, such as enhanced care in homes and remote consultations, while working within the hub to deliver flexible, patient-focused care, whether through face-to-face appointments or telephone triage. About us Location : Stalybridge, Dukinfield & Mossley PCN Contract : Part-time, 5 sessions per week Experience : Newly Qualified GP (Under 2 Years) Job Description Overview: We are seeking a dynamic and motivated GP to join our innovative hub-based model, working across 9 GP practices within the Stalybridge, Dukinfield & Mossley Primary Care Network (PCN). This role offers an excellent opportunity for a newly qualified GP to deliver high-quality patient care while gaining diverse clinical experience and professional development in a supportive environment. Key Responsibilities: Patient Consultations : Conduct consultations with both acute and routine patients across the 9 practices within the PCN. Manage a varied caseload, including minor ailments, chronic diseases, and undifferentiated medical conditions. Ensure patient care adheres to the clinical guidelines and practice protocols. Referrals : Assess and triage patients, making appropriate onward referrals to specialist services or secondary care when necessary. Collaborate with referral management teams and other healthcare professionals within the PCN to ensure seamless patient care. Prescription Management : Review and process prescriptions efficiently, ensuring compliance with best practice and clinical guidelines. Support other healthcare professionals in the PCN with medication reviews, repeat prescriptions, and resolving medication-related queries. Collaboration with PCN and Extended Teams : Work as part of a multi-disciplinary team that includes nurses, pharmacists, healthcare assistants, physiotherapists, social prescribers, and mental health professionals. Provide GP input and leadership to ensure integrated care across the PCNs services. Participate in team meetings, case discussions, and quality improvement initiatives. Extension of Practice Services : Participate in initiatives that extend GP practice services, such as enhanced care for care home residents, remote consultations, and supporting ARRS professionals. Adapt to patient needs by offering flexible service delivery via face-to-face appointments or telephone triage within the health hub or across the practices. Skills and Qualifications: GMC registered GP with a license to practice in the UK. Newly qualified GP (under 2 years of experience) with a strong interest in primary care. Excellent communication and consultation skills. Ability to work collaboratively within a multi-disciplinary team. Strong organisational skills with the ability to manage a varied caseload.
