Ealing Cognitive Impairment and Dementia Services, within the Older People's Mental Health Service offers a hugely exciting opportunity to be part of a supportive, forward thinking, adaptive team. We welcome applications from nurses on all parts of the NMC register with the required experience and skill set. We are looking for dedicated, enthusiastic nurses who are highly motivated and caring professionals who can provide the best possible care with compassion to our patients. You will have post registration experience, preferable, in the specified care group, and will be someone who can make a significant contribution in supporting the delivery of high quality assessment, investigation and treatment, supporting the team in the day-to-day running of the service whilst being expected to take a lead on a core aspect of service delivery. The post holder will work with individuals who present with a range of memory related health needs, by providing assessment, information, treatment, review and support through individual and group work as part of a multi-disciplinary team. You will work as a Care coordinator for a group of patients, and be responsible for liaising with other professionals and organisations to ensure that patients' physical, social and psychological needs are met in an integrated and holistic way. This is a fulltime post (WTE 1.0) working Monday to Friday, 9-5 hours. The post holder will provide specialist assessments, reviews and nursing interventions to service users accessing the Cognitive Impairment & Dementia Service (CIDS). The post holder will ensure the successful implementation and roll out of care for dementia patients Main duties include but are not limited to: Specialist assessment of individuals and their families Assessment and management of ongoing risks as identified during the assessment, ensuring that this is done in collaboration with service users and carers. Provide supervision of junior staff and trainees where appropriate. Work as a core member of an MDT. Act within the NMC Code of Conduct at all times. Deliver care under the Care Programme Approach and work in partnership with individual service users and their families and carers to ensure the delivery of the care plan. Administer and monitor medication, including depots, prescribed to individual service users, providing information and advice, within NMC guidelines and the Trusts Medicines Code. Broaden research and development skills through participation in local audit and research projects.
