Aylesbury Crisis and Home treatment Team is part of Urgent care Pathway service base at Whiteleaf centre in Aylesbury We are looking to recruit an enthusiastic support worker who is passionate about working with people experiencing a mental health crisis in their homes and who would like to develop their skills in delivering evidence-based interventions. You will work as part of the Crisis Resolution Home Treatment Team supporting patients in the community to avoid hospital admission. The post holder will support the delivery of care specified within the plans of care formulated and directed by mental health practitioners and the broader multidisciplinary team for those patients who are in a mental health crisis and require intensive home treatment and support without which they would likely require admission to Hospital. You will have the opportunity to work alongside a range of practitioners. Clinical supervision is provided along with opportunities for training and continued professional development. We are looking for someone who wishes to develop their skills in a mental health service. You are likely to have experience of working with people with mental health problems. You will have a supportive team around you and a clinical supervisor to work with as well as training and development opportunities to develop your skills and experience. To provide assistance to interventions and elements of planned programmes of care to service users under the direction and clinical supervision of a clinical lead To ensure services are delivered in a manner that is appropriate, relevant and respectful to all community members, recognising the diversity of ethnicity, culture, belief, privilege and capacity of service users and carers To have a flexible approach in providing support, including administrative tasks, duties and activities as and when required across the service To effectively communicate routine information and advice to service users, other professionals, and external agencies. To be responsible for the accurate maintenance of clinical record keeping and the required communication of individual patient care Carry out delegated care tasks including supported meals and skills sessions. Observe and report any changes in patients' conditions to the key Nurse and the team. Liaise with other relevant individuals involved with the patient, as appropriate. Write accurate records of care activities and communications with patients, relatives and carers in the patient's notes. Support patients with individual needs and choices. Able to empathise, be supportive and sensitive to the needs of others as well as being able to cope with distressing circumstances Able to work flexibly on a shift pattern to cover 24hrs of CRHTT Service as required and the ability to travel independently between locations to fulfil the requirements of the position.
