                                                     We are seeking an experienced Practice Nurse with a minimum of one year experience working in Primary Care, and a qualification in diabetes or a keen interest in developing expertise in this area. The successful candidate will play a crucial role in managing and supporting patients with diabetes, contributing to their overall health and well-being. They will work as a key part of the clinical team and must be able to organise and manage their own workload.     Treatment room duties to include venepuncture, wound care, administration of injections and immunisations, carrying out ECGs, removal of sutures and staples, advice on ear care and carrying out ear irrigation where appropriate, urine sampling.   Support and supervise trainees with treatment room duties in accordance with their competencies.    Carry out cervical screening where postholder holds a current smear takers code.    Provide reviews for patients with long term conditions based on QOF. This may include diabetes, hypertension, cardiovascular disease, chronic kidney disease, learning disability and mental health reviews.    Provide high-quality nursing care and support to patients with diabetes.   Conduct diabetes clinics, including assessments, education, and management plans with the support of the Diabetes Nurse Lead and GP Lead.   Collaborate with GPs and other Healthcare professionals to deliver integrated care.    Participate in health promotion activities and patient education. Integrate health promotion and patient education within annual reviews.    Maintain accurate and up-to-date patient records using EMIS.
