About us Your Key Responsibilities Optimising Medicines & Supporting the Practice Team Reduce GP workload by taking the lead on medicines-related queries, prescribing reviews, and medication adjustments. Drive improvements in prescribing safety and quality , ensuring compliance with best practices and regulatory guidelines. Provide expert support to the wider practice team , offering guidance on complex prescribing decisions and medicines optimisation. Be a trusted point of contact for patients , ensuring they have access to safe, effective, and appropriate medication. Lead on antibiotic stewardship and medicines safety , supporting practice-wide initiatives to enhance patient care. Clinical Duties Medication Reviews Conduct structured medication reviews, particularly for patients with polypharmacy, frailty, or multiple long-term conditions . Chronic Disease Management Support patients with hypertension, Type 2 Diabetes, asthma, COPD , and other long-term conditions. Medicines Reconciliation Ensure safe, effective transitions of care by reviewing medications when patients move between providers. Care Home & Frailty Reviews Work proactively to reduce inappropriate prescribing and optimise medication regimens for vulnerable patients. Dispensary Leadership & Development Oversee and develop the practice dispensary , ensuring it operates efficiently, safely, and in line with regulations. Support and manage dispensary staff , providing mentorship, training, and ensuring compliance with audits and best practices. Drive quality improvement initiatives , identifying opportunities to enhance efficiency and patient experience in the dispensary.
