District Nurse Clinical Specialist with Hampshire and Isle of Wight Healthcare NHS Foundation Trust in Sandown, Isle of Wight
We are recruiting for a District Nurse Clinical Specialist to come and join our team here on the beautiful Isle of Wight. Part time (30 hours per week) or Full time (37.5 hours per week) will be considered. The District Nurse Clinical Specialist is a pivotal member of the Integrated Community Nursing Service, responsible for delivering and leading specialist clinical care for patients on the District Nursing caseload. In doing so, the post holder will be expected to assess, plan, deliver and evaluate patient centred programmes of care ensuring that they practice within their sphere of competence and knowledge. You will have the flexibility and skill set, to work across planned pathways and with specialist services, supporting patients to live or die well in the place that they call home, and prevent unnecessary hospital admissions in collaboration with Virtual Ward. The successful candidate will be required to undertake relevant training events at any site across the trust. In periods of high activity or/and when clinical areas require additional support this will be risk assessed by the relevant Locality Manager who will consider the impact on the individual's workload and the relevant service. This post is required to work in any location across the three localities. Previous applicants need not apply Duties will include: * Lead and manage a team to deliver care in the home and community. Be responsible for developing new and innovative clinical practices, responding to emerging knowledge and techniques working with other services where appropriate to make improvements * Work in an integrated and partnership way within the Integrated Localities, primary, secondary, social care, the independent and voluntary sector, and others to improve the health and care of individuals, families, and communities, particularly the most vulnerable reducing health inequalities. * Work to professional and regulatory body codes, standards and guidance at all times ensuring clinical practice is grounded in evidence based theoretical and practical knowledge. * Work in partnership with the wider Community Services Division taking a lead with the quality and governance agenda. * Act with autonomy and authority, making decisions within an agreed professional and managerial structure. * Create and promote a learning culture within community nursing to support the professional growth and development of nursing staff, students and as appropriate members of the multi-disciplinary team * Provide information, education, support and advice to patients, families, carers and health care professionals about aspects of care relating to Community Nursing and Locality Therapy About us Further information about the Trust and this role can be found on the Job Description and Person Specification document attached.
