                                                     The Community Nurse Specialist (CNS) Access Team Leader is an expert in ensuring high quality palliative care is delivered to patients and their families. They work independently, and as part of the wider health care team. We are a progressive and dynamic community nurse specialist team at the forefront of delivering outstanding palliative care. Due to a recent service restructure, we are looking for a passionate and enthusiastic individual to lead the dedicated team of community nurse specialists managing our advice line. The successful candidate will primarily support patients via the advice line, but also have the ability to responsively visit patients at home when urgently required. To apply for the role you will need to be a non-medical prescriber with a minimum of 2 years specialist palliative experience. We would also welcome applications from candidates with a minimum of 18 months experience in a specialist palliative role, evidence of leadership / management and a willingness to undertake the independent prescriber module. The Community Nurse Specialist Access Team support any palliative patient, family and health care professional (within BNSSG) contacting the hospice advice line within the hours of 07:45 to 20:00 Monday to Sunday including bank holidays. We can offer eitherfull orpart time hours (37.5 or 30 hours per week)   About us  Clinical practice 1. Act as a clinical resource for less experienced / non-prescribing colleagues 2. Manage and coordinate the advice line, supporting patients living in the community 3. Take and analyse a clinical history in a relevant succinct and logical manner 4. Independently assess the patients needs, identify their priorities and work with the patient and family towards achieving them 5. Use physical assessment and clinical reasoning skills as part of patient assessment and to support clinical advice, professional communication and documentation. 6. Perform Independent Prescribing within area of competence once registered as an Independent Prescriber with the Nursing and Midwifery Council and practice according to local policy. 7. Demonstrate an in-depth knowledge of the options for achieving control of symptoms and ability to advise the primary health care team 8. Demonstrate an ability to explore the personal resources of families/carers in order to facilitate, where possible, that patients are cared for in the place of their choice 9.
