Paediatric Advanced Clinical Practitioner - Ambulatory Unit with King's College Hospital NHS Foundation Trust in London
This is a very exciting opportunity to join an innovative and friendly Paediatric Ambulatory team. We are looking for proactive, enthusiastic, caring and compassionate individuals with excellent leadership and communication skills. The Paediatric Advanced Clinical Nurse Practitioner will work in conjunction with the Paediatric Consultants within the ambulatory unit to provide expert care for patients that undergoing day case and ongoing medical treatment plans. With clinical support and appropriate training there will be opportunities to develop skills and carry out extended roles that are relevant to the Paediatric Ambulatory Unit. In conjunction with the medical team in the Paediatric General Medical Department provide expert care for a complex caseload of patients, accepting professional and legal responsibility and accountability for all aspects of own work With clinical support and appropriate education/training, to develop skills in and carry out extended roles including: o Advanced Clinical Assessment of paediatric patients o Making referrals to other healthcare professionals o Insertion of PICC lines o Performing Lumbar Punctures o Independent Prescribing including blood products To be an active leader within Paediatrics and contribute to strategic planning within the department To contribute to the education of the multi-disciplinary team and departmental research, audit and quality improvement programmes as appropriate About us Clinical Practice/Care Delivery To demonstrate clinical expertise and a high level of complex decision making, with evidence of increasing autonomy, clinical judgment and decision making skills To accept clinical responsibility for a diverse and often complex caseload of patients, ensuring the use of best evidence to make clinical decisions, complex treatment plans and evaluate these to manage acute and chronic illnesses With appropriate support develop own skills to include a. Advanced clinical assessment skills b. Insertion of PICC lines c. Lumbar Punctures d. Independent Prescribing including blood products To arrange and order timely investigations e.g. blood tests, X rays, collate and where appropriate interpret these, ensuring prompt action on the results. To contribute to the education of patients about their condition, diagnostic tests and treatment options, enabling them to make an informed choice. To liaise with Ward Sisters/Charge Nurses, administrative staff, and other members of the MDT to optimise the smooth transition of paediatric patients through the treatment pathway to discharge home. To support timely discharge processes, complete discharge summaries and TTA drugs. To collaborate with ward staff regarding referral to GPs, Community Nurses and other medical specialities as necessary. To organise own workload efficiently and effectively with regard to clinical priorities and use of time. To be highly visible, accessible and approachable to staff, patients and the public to ensure that open and honest communication channels are created and sustained. Using advanced communication skills, to ensure effective communication with a wide range of people in a manner consistent with their level of understanding, culture and background and preferred ways of understanding To maintain an overview of the standards of care being delivered across defined clinical areas and instigate remedial action if necessary. Maintain written records of treatment plans consistent with the outcome of assessment, maintaining accurate and legible patient notes (written and electronic) in accordance with Trust and national professional policies and guidelines. To generate concise, comprehensible and accurate letters and reports as required.
