Occupational Therapist with Livewell Southwest in Plymouth, Devon

This project forms part of a wider plan to improve urgent and emergency healthcare delivery. We are committed to revolutionising healthcare delivery for frail and elderly patients by launching a Community Frailty Virtual Ward. Our aim is to provide high-quality, patient-centred care in the comfort of their own homes, reducing hospital admissions and length of stay in hospital and enhancing overall well-being through enhanced care coordination. Interviews will be held at the Beauchamp Centre, Mount Gould Hospital. As Band 6 Specialist Occupational Therapist you will play a critical role in our new multidisciplinary team, covering one or more of the 6 new frailty virtual wards that we have planned for our locality. The post holder will work closely with consultants, pharmacists, dietitians, and other healthcare professionals to ensure seamless, integrated care on the virtual ward, by providing specialist OT assessments and interventions for those admitted to the Frailty Virtual Ward. Key Responsibilities Work with the wider MDT to provide specialist OT assessments and interventions To provide a specialist, high quality occupational therapy service within Frailty Virtual Wards. To provide specialist occupational therapy assessment, diagnosis, treatment and education to individuals admitted to the Frailty Virtual ward who may have a range of conditions and presentations. About us To provide specialist occupational therapy assessment, diagnosis, treatment and education to individuals admitted to the Frailty Virtual ward who may have a range of conditions and presentations which could include neurological diagnosis and symptoms such as stroke. To provide specialist occupational therapy assessment, diagnosis, treatment and education to individuals admitted to the Frailty Virtual ward who may have a range of conditions and presentations. To provide advice, signposting, guidance and information to health and social care professionals, patients, carers, relatives, or other non-professional contacts. To be an integral member of the MDT providing education, training and dissemination of information to all healthcare professionals. To be an autonomous practitioner, holding a caseload and working without direct supervision. To continue own professional development in line with HCPC regulations. Work as an outpatient and community-based practitioner undertaking an active role in planning, developing, coordinating, delivering and participating in the evaluation of Frailty Virtual Wards. Carry out comprehensive specialist assessments for patients with neurological and stroke presentations, including those with diverse or complex presentations/multi-pathologies, to identify their problems and needs and ensure that patient's holistic needs are considered. Formulate and deliver individual treatment programmes based on specialist knowledge, evidence-based practice, national and local guidelines using profession-specific clinical skills and techniques. (e.g. 1:1s, group activities, patient education, carer education, use of electrical stimulation techniques, cognitive and vocational rehabilitation). To discuss and agree the assessment outcomes with patients and facilitate goal setting where appropriate. Demonstrate ongoing evaluation of client progress using agreed outcome measures. To work closely with the specialist multidisciplinary team contributing to individual patient care e.g., MDT meetings, case conferences. Provide advice to other professionals/agencies to meet patients holistic care needs. Undertake evidence-based audit and research projects to further own and team clinical practice. May lead the implementation of specific changes to practice or contribute to service protocols. Supervise, educate and assess performance of students and junior staff, in order to provide expert advice and clinical support whilst promoting the effective and efficient delivery of a high-quality service.
