Advanced Clinical Practitioner for Frailty Services with Royal Surrey NHS Foundation Trust in Guildford
This is a generic Job Description for Advanced Clinical Practitioners (ACPs) working within the Trust. Specific responsibilities will vary between specialties, however all ACP posts will be expected to reflect the four pillars of advanced practice: Clinical Practice, Leadership, Education of self and others, and Research & Development. a) Work to a high level of personal and professional autonomy, using specialist knowledge and skills to deliver independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment, health promotion and safe admission and discharge of patients. This may include:- on-call/out-of-hours and in-hours/bleep-holding responsibilities, as part of the agreed job plan. Act as an independent non-medical prescriber, where applicable to role/profession b) Work collaboratively with all members of the multidisciplinary team to provide best clinical care, emotional and psychological support and education to patients and their carers. c) Facilitate evidence based practice, and promote and undertake research and clinical audit within the service and across the Trust. Design and provide clinical/practical and theory-based education and training, eg. to nurses/AHPs, junior medical staff. d) Work with innovation and flexibility to adapt to a fast-changing specialty, and play an integral role in the operational/strategic development of the service. The Advanced Clinical Practitioner (ACP) will provide expert assessment, treatment and management of a case load, using complex communication skills, judgement, discretion and decision making. They will monitor and assist with improving standards of care, supervising, teaching and supporting colleagues providing skilled professional leadership. Where appropriate they will act as an expert in their field and lead and/or participate in initiatives to improve standards of care for the client group and assisting to advancing the Frailty ACP role. The Advanced Clinical Practitioner is service driven, and the role is structured around the Department of Health Guidance across the four domains of Advanced Practice, those being: 1. Clinical Practice 2. Leadership and service development 3. Research 4. Education About us We have an exciting opportunity for an Advanced Clinical Practitioner (ACP) to work in our Integrated Frailty Crisis Pathway. This role will initially sit within our community based integrated service, rotating through our Single Point of Access and integrated Urgent Community Response/ Hospital at Home service. There will be scope to rotate through other aspects of the Acute Frailty Service including the ED in reach team, Frailty Same Day Emergency Care and inpatient Older Peoples Advice and Liaison. We welcome applications from both Allied Healthcare Professionals and those with a nursing background who meet essential criteria. Within the Royal Surrey Frailty services, we have expanded our ACP workforce to help deliver excellent care to all older people with Frailty. We have several established posts within the Acute Frailty Team (AFT), Frailty Same Day Emergency Care (FSDEC), Urgent Community Response (UCR) and Hospital at Home (H@H). These posts contribute to the weekend rota for H@H. With time we hope to be able to rotate trainee ACPs through several services to give them a broader training experience. You will be required to work as a semi-autonomous clinician within the service and will be supported in your training through our ACP programme aligned to the Frailty Academy. You will have a dedicated clinical supervision and will be supported in your professional development in order to fulfil your on-going commitment to the 4 pillars of Advanced Clinical Practice. We provide a 7-day service and it is likely you will have to contribute to weekend and evening rotas.
