Applications are sought from Nursing, Allied Health Professional, Psychology and Social Work Candidates holding a suitable foundation degree level 4 or level 5, for an exciting opportunity to work within the In-patient to Community Transition Team. The appointed candidate will be offered a substantive position, working 37.5 hrs per week. The post-holder will have experience working with individuals who have a mental disorder, personality disorder, have had contact with the criminal justice system and knowledge of the collaborative process of recovery. This role will require travel between in-patient and community bases so excellent diary management, and good delegation skills are required. Duties will include supporting qualified staff to complete a range of specialised assessments including in-patient referrals to Westbridge and the Community Teams, Mental Capacity Assessments, Care Act reviews and Financial Assessments. Supporting the multi-disciplinary team, providing trauma informed care, assisting in a range of assessment-based interventions. Attending and contributing to CPA reviews, Professional's Meetings, and Discharge Planning Meetings. Direct patient care identifying and accessing supportive networks, occupation, leisure activities, community healthcare providers, substance misuse education and support providers. Delivering treatment and therapies to support individuals' transition and living meaningful lives in the community, for example, substance misuse work, well person and trauma informed care. Submitting applications for benefits, supported accommodation, residential and nursing care placements Effective and timely communication with individuals and families as part of the discharge process. Communicate and complete records for individuals.
