Healthcare Support worker/Enablement Assistant with Hampshire and Isle of Wight Healthcare NHS Foundation Trust in Southampton, Hampshire
We are looking for staff to assist the qualified team to provide specialist healthcare, neuropsychiatry, rehabilitation, and enablement services to adults with a cognitive impairment on our Kite ward. This is achieved through a person centred care approach, which is comprehensive and responsive to the needs of the individual and their family/carers. Identified needs will be assessed, treatments implemented and evaluated in partnership with service users as well as family/carers. To provide high standards of effective health care for people accessing the assessment service. This includes intimate personal care, daily domestic duties, recreational activities, educational opportunities, and the management of financial affairs and a full range of everyday living requirements. This role can be part of a career pathway, which includes senior enablement assistant, nursing apprenticeships and associate practitioner roles. Actively promote recovery focussed care with patients, in conjunction with other health professionals, including achievement of set goals. Support patients with their rehabilitation and recovery, respecting them as individuals, supporting them and understanding their right to express their needs. Accurately assess, describe and record observations on progress or change in both patients' mental and physical state, using appropriate assessment tools. Escalating any concerns as appropriate. Undertake clinical and environmental risk assessments and document key risk areas, seeking guidance from more senior staff when dealing with cases indicating a high level of risk. To raise incidents appropriately via the reporting system in an accurate, objective and concise manner. Provide information on improving health and wellbeing and signposting to other agencies as appropriate.
