Mental health services in Manchester are working towards Community Transformation; with a new framework the vision is for a place-based community mental health model. We recognise that social work plays a key role within this. Social work is critical in upholding individuals' human rights, fostering a strengths and asset based approach to an individual's mental health recovery; and a component to multi-disciplinary integrated working. As such, we are looking for a Social Worker who is able to take up a role in supporting our service users in delivering high quality, person centred, strength based social care duties, through empowering assessments and support for individuals within our Specialist CMHTs in North Manchester. We have bimonthly AMHP and social work forums. You will have access to social work specific supervision delivered by an experienced social worker. We currently have a position at North Manchester Specialist CMHT based in our exciting new and modern base at the Central Park Hub. You will be responsible for carrying out social work duties is a fast paced multi-disciplinary team requiring the highest levels of skills, knowledge and professional expertise. You will ensure the delivery of effective, safe integrated services for adults through the promotion of high standards of practice in assessments and person-centred planning, being creative to meet individual outcomes. The role involves: Assessing, planning and review in line with requirements of the Care Act (2014) and s117 of the Mental Health Act (1983). Taking a strengths based and assessed based approach to support Work with a caseload of individuals in a collaborative way to support in the development of their assessments, recovery plans and structured care plans Signpost to non-statutory services or commission packages of care. Complete high quality assessments of Mental Capacity (in line with Mental Capacity Act 2005) and chair Best Interest Meetings Make safeguarding enquiries in line with s42 of the care Act (2014) ensuring that safeguarding is person centred, outcomes achieved and risk mitigated Comply with SWE Professional Standards and be Registered with Social Work England , Comply with legislation, national and local policies and procedures. About us To demonstrate a high level of active communication skills with individuals and also in a variety of forums and situations. To utilise your highly developed interpersonal skills to facilitate effective communication. This includes identification and overcoming communication barriers such as sensory or cognitive impairment, hostile or highly emotive situations.. With support from line manager, make safeguarding enquiries in line with s42 of the care Act (2014) ensuring that safeguarding is person centred, outcomes fully explored and risk mitigated To act inline with the Trusts Infection Prevention Control policies, procedures and best practice guidance to maintain high standards of Infection Prevention and Control (IPCC) To comply with all service security requirements with regarding to your own and others safety. These include lone working procedures, H&S at work and risk management procedures. To ensure the management of risk in accordance with the Trust Policies and procedures including the investigation process , complaints accidents and near miss situations via the In phase system. To comply with all health and safety legislation and the welfare of yourself and the wider Trust. To maintain your personal well being by the use of all available resources available to you via the Trust or professionally. To maintain your personal and professional duty of candour where an incident or accident has occurred. To develop and maintain good working relationships with all partners involved with individuals care or the wider health economy including General Practitioners, commissioners and third sector agencies. To promote awareness of mental health and the role of the CMHT within local services, primary care services and also within the wider community. To maintain your professional registration and to inform the Trust if there are any changes or restrictions on practice active on your registration. To act up for Senior Practitioner if required To contribute to the delivery of multidisciplinary training including the induction of new staff, students and trainees. To act as a Practice Educator for social work students on placements if qualified. To encourage staff to work towards their Practice Educator Qualification once qualified. For those experienced to act as supervisor for NQSWs completing ASYE. To maintain Professional Registration To keep practice up to date and record how the use of research and theories informs interventions. To undertake any other reasonable duty, when requested to do so by an appropriate Trust manager. To understand and comply with all Trust policies, procedures, protocols and guidelines. To understand the Trusts Strategic Goals and how you can support them. To understand the need to safeguarding children and vulnerable adults and adhere to all principles in effective safeguarding.
