                                                     Churchmere Medical Group is a rural 21,000 patient practice in Ellesmere and Whitchurch, within easy commute of Chester, Wrexham, Crewe and Shrewsbury areas. An exciting opportunity for a Nurse Practitioner has arisen to join our team, either full time or part time. We are looking for a flexible and reliable nurse with excellent interpersonal and clinical skills. You will have previous experience of telephone triage and assessment of minor illness/injury. The ability to independently prescribe would be desirable but not essential. Experience in chronic disease management as well as baby immunisations, cervical smears, dressings and injections is desirable however full training can be provided. The successful applicant will work across all sites and collaboratively with our multidisciplinary team to deliver patient care. The successful candidate will be required to complete an enhanced DBS check. Salary is negotiable and dependent on experience.   The post holder is a caring, committed and experienced prescribing nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment, diagnostic skills and prescribing, for patients within the general practice. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. About us  Job responsibilities: Diagnosing and treating patients presenting with minor illness:   Triage and treat patients wishing to see a health care professional using a variety of techniques to elicit and interpret the history of an event/illness, including past medical history and drug history in order to reach a clinical decision, provide appropriate care and/or make any necessary referrals to other members of the primary health care team.   Disseminate test results to patients and counsel them accordingly.   Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations. Management of Chronic Disease:   Contribute to chronic disease management by assessing, planning, monitoring and reviewing progress.
