In this position, the Health Visitor is expected to apply their specialized skills and knowledge to deliver evidence-based care, develop health profiles for local populations, and identify public health needs. Key activities will include coordinating parenting groups, performing health assessments, promoting health education, and implementing initiatives that target specific health issues within the community. The role is critical in enhancing the health and well-being of individuals and families across various community settings.
o To plan, organize and evaluate an innovative health visiting service, and where appropriate,delegate to other members of the Health Visiting team to ensure delivery of a quality serviceo To deliver the healthy child programme and work in partnership with families to develop andagree tailored health plans to address their parenting and health needs.o To actively participate in service development activities.o To work in collaboration with other health care and service providers to ensure seamlessdelivery of services for clients and communities
