Join our compassionate West District Nurse Community Team as a Clinical Coordinator in stunning York, where your communication skills can shine and your dedication to patient care will truly make a difference. In this pivotal role, you’ll be the friendly point of contact for patients, carers, GPs, and fellow team members, expertly triaging referrals and allocating tasks to ensure seamless care delivery. We understand that conversations with patients can sometimes be sensitive, so your empathy and patience will be invaluable as you support individuals in their most anxious moments.

This position offers a dynamic and supportive work environment, with flexible shifts primarily from Monday to Friday.
