The Tees Esk and Wear Valleys NHS Foundation Trust is seeking a full-time Community Associate Practitioner for the Harrogate & Ripon Crisis and Home Treatment Mental Health Team, based in Ripon. This newly created role involves collaborating with a multidisciplinary team, including psychiatrists and crisis clinicians, to deliver physical health monitoring and support for service users with diverse mental health needs. The candidate will conduct physical health checks, such as blood sampling and nutritional advice, particularly for those on specific medications, while also taking the lead in providing assessment and support for carers of individuals receiving treatment.

Applicants should have a Foundation Degree in Health and Social Care and equivalent literacy and numeracy skills. The position requires independent travel across the locality, and considerations for adjustments may be available under the Equality Act 2010.
