This post provides a range of practical and educational opportunities for 4-months in Geriatric Medicine. The post holder will develop a variety of generic and core specialist competencies that focus upon the appropriate skills, knowledge and attitudes required in order to ensure high quality and safe care is experienced by every patient. This is valuable post for those doctors seeking NHS experience.
Responsibilities
As a Foundation Year 1 (F1) Doctor, you will provide high-quality clinical care under supervision, ensuring patient safety and effective team collaboration. Your responsibilities will include:
Conducting patient assessments, investigations, and initial management plans.
Supporting ward rounds, documenting patient progress, and liaising with senior colleagues.
Participating in on-call duties, handovers, and responding to acute medical situations.
Engaging in clinical audits, quality improvement projects, and teaching opportunities.
Ensuring adherence to GMC guidelines, hospital policies, and safe prescribing practices.

- Promptly assess airway, breathing and circulation in a collapsed patient and take blood pressure using automated methods or with a sphygmomanometer.
- Identify and respond to acutely abnormal physiology. Administer oxygen, monitor efficacy, correct hypotension and identify oliguria and intervene appropriately.
- Where appropriate deliver a fluid challenge safely to an acutely ill patient by selecting, setting up and giving fluid bolus(es), identifying hypokalaemia and choosing a safe & effective method of potassium supplementation.
- Reassess acutely ill patients promptly following initiation of treatment by implementing a system of regular checking of unstable patients and seeking appropriate help of patients that do not respond to initial measures.
- Request senior or more experienced help when appropriate after firstly analysing the clinical problem(s) and considering possible causes and solutions.
- Undertake a secondary survey to establish differential diagnosis, recognising the importance of iterative review.
- Obtain arterial blood gas samples safely and interpret results correctly.
- Manage patients with impaired consciousness including fits and following local protocols.
- Safely use common analgesic drugs after evaluating the patient in pain and prescribing opioid and non-opioid drugs safely and re-evaluating the efficacy of analgesia in a timely manner. Monitors patients for common side-effects and safely use anti-emetic drugs when appropriate.
- Manage patients following self-harm whilst demonstrating tolerance and understanding. Take focused history including psychosocial causes requiring social services or police intervention and perform a mental assessment. Assess toxbase, recognise the need for involvement of mental health or more experienced people and demonstrate an awareness of child protection concerns where appropriate.
- Manage a patient with acute psychosis following initial diagnosis and administer appropriate anti-psychotic drugs.
- Ensure punctual and safe continuing care of patient between shifts/on call staff by providing clear information to colleagues and accurately summarising the main points of diagnosis, active problems and management plan.
- Consider the appropriate interventions according to patient's wishes by extracting information from the history and examination that would influence treatment intensity decisions, seek information from relatives if appropriate and consider factors including the use of national and local do-not-resuscitate procedures, ensuring a balanced view of benefits and harms of medical treatment.
- Demonstrate life support skills to intermediate life support standard.
- Participate in acute admission management with appropriate organisation and understanding of role(s) by attending hand-over, accepting direction/advice and allocation of tasks from seniors. Prioritise and seek timely advice from colleagues.
- Request and deal with common investigations appropriately for patient's needs and communicate the risks, possible outcomes and later results with patients (carers). Recognise normal and abnormal results and prioritise the importance of these.
Demonstrate competency in venepuncture and cannulation, blood cultures from peripheral and central sites, intravenous infusions including the prescription of fluids, blood and blood products. Perform an ECG, arterial blood sampling, injections - subcutaneous, intradermal, intramuscular and intravenous, urethral catheterisation, airway care including simple adjuncts and nasogastric tube insertion.
