The Hospital @ Home Team Lead position at Lewisham and Greenwich NHS Trust is a one-year fixed-term role focused on delivering high-quality nursing care to paediatric patients in the community while facilitating early hospital discharge and preventing unnecessary admissions. The successful candidate will act as an autonomous expert practitioner, leading a team of nurses to provide seamless care, manage patient caseloads, and collaborate with multidisciplinary teams to uphold family-centered practices. Responsibilities include clinical assessments, developing care plans, providing mentorship and training to junior staff, implementing best practices through research and audits, and managing administrative tasks related to the team's operation.
