We have a fantastic opportunity for an Enhanced Clinical Practitioner (ECP) or Trainee Enhanced Practitioner to work in the Swadlincote Acute Home visiting service.
We will positively consider applicants who fulfil the Person Specification, with the exception of non-medical prescribing and advanced clinical decision making/patient examination skills.
However, we will consider those applicants who do not meet this criteria and are willing to accept a Band 6 Trainee Enhanced Practitioner role until successful completion of both programmes and an additional portfolio of enhanced skills.
The role will be either Band 6 or Band 7 (dependent on experience and qualifications) .
The Acute Home Visiting service works collaboratively with the local GP and Integrated Teams in Swadlincote which provides a varied source of support. Our patient group is patients in their own homes and in the Oakland bedded unit which supports patients discharged from hospital.
Our team is happy and skilled and provide a really good working environment with opportunities to learn and develop in this role.
'The 2024/25 pay award is due to be reflected in pay from October 2024, with any arrears due paid in November 2024'

We want to support patients accessing help when needed and optimising health outcomes.
We also want to support patients to remain in their own homes as appropriate preventing unnecessary admission to hospital and also support patients to return home as needed.
