The role of the Care Programme Approach Coordinator requires the post holder to demonstrate excellent communicate skills, have well developed IT and time-management skills; a proficiency for problem solving and an ability to priorities their own duties and tasks.

An exciting opportunity has arisen within the team of existing of Care Programme Approach (CPA) Coordinators. This post is hosted within the Directorate of Forensic Mental health and Learning Disabilities The role will include ensuring that the essential elements of CPA, as outlined in Scottish Government guidance is observed by individual professionals, clinical teams and external agencies in line with agreed inter-agency operational procedures. It will also include ensuring the effective involvement of patients, carers and all stake holders in assessing, planning, implementing and evaluating programmes of care. The Care Programme Approach (CPA) is designed to ensure that individuals (aged 16 years and over) who have severe and enduring mental illness (including dementia) with complex health and social care needs receive the appropriate continuing care and supervision. CPA is also applicable to individuals who have a learning difficulty and meet the specified criteria. It is also mandatory for all Restricted Patients.

The Care Programme Approach Coordinator will be required to produce a clear, concise and reflective account of the clinical outcomes and decisions agreed at Care Programme Approach meetings, in incorporating the ability to comprehend complex and sensitive material, critically analyse data, and recording it in a format that is universally understood by professionals, patients and carers. The post-holder will be responsible for the co-ordination of the Care Programme Approach Service, within their own geographical defined area within NHSGG&C. They will play an active/lead role in the day to day management of the service, procedures and guidelines, under support and supervision of the Service Lead Additional aspects of the role include the provision of ongoing advice and support to individual professionals and teams involved in the delivery of care and essential services to the target patient group on matters related to the functioning of the Care Programme Approach. Relevant advice is likely to cover areas such as; identification of needs, the development of needs based multi-disciplinary care plans and provision of guidance related to the role and function of external agencies who contribute to Care Programme Approach.

Previous experience of data input/electronic database is also essential with an advanced level of competence in the following Microsoft Office applications: Microsoft Word, Excel, PowerPoint and Access is also essential. Currently the post is a blend of Teams meetings and also face to face which will involve travel to various sites and buildings across the city to accommodate this. These will be planned with the appropriate clinical leads for the patient and with what is appropriate for the person at the time.
