In accordance with individual support plan, enable the service user(s) to look after and clean their own rooms, organise their personal possessions, clothing and other personal effects.

MAIN PURPOSE

  • To provide support to our service users appropriate to their individuals needs ensuring that each service user leads a full and valued life.
  • To contribute to providing a homely environment, whereby the service user has maximum involvement in their own activities and decision making.
  • To enable service users to participate in and contribute to the local community.

MAIN TASKS AND RESPONSIBILITIES
  • Establish and maintain professional working relationships with the service users, contributing to the creation of a homely and pleasant living environment for each service user.
  • During the course of your work you will at all times conduct yourself in a professional manner acting as a role model to colleagues and service users contributing to the delivery of quality services. To work as a member of the team, contributing to the team ethos and supporting service users(s) and colleagues appropriately.
  • Facilitate the daily routines of the service users including personal care promoting a high standard of personal hygiene at all times.
  • Participate in the development of service user(s) support plans, risk assessments and programmes, complete written records as necessary and in accordance with the guidelines provided.
  • Implement individual support plans to meet the needs of the service user(s). Highlight any issues/concerns to the Team Leader to ensure the service user(s) receive quality services at all times.
  • Participate and contribute as required to individual service user review meetings and support plans within boundaries set by senior colleagues.
  • In accordance with individual support plan, enable the service user(s) to look after and clean their own rooms, organise their personal possessions, clothing and other personal effects. Assist in the cleaning and maintenance of all rooms, fixtures and fittings thereby maintaining a safe environment for service users and staff.
  • Enable service user(s) to access and utilise the resources and activities available within their local and wider communities. Accompany service user(S) as required, ensuring they get to and from activities safely and promptly.
  • Accompany service user(s) on activities and actively participate as required.
  • Act as a Keyworker for designated service user(s) liaising closely with relevant staff regarding the individual programmes to ensure quality services are delivered at all times.
  • You are required to report any enquiries /issues and/or concerns to the line manager providing information to enable them to take appropriate action.
  • To liaise in a professional manner at all times with service user(s) family members, friends, advocates and other professionals as required at your location.
  • To ensure that all relevant documentation is completed accurately and effectively in a timely manner.
  • To attend and contribute to annual reviews, multi-disciplinary meetings (MDT) and Person Centred Planning meetings and all other relevant meetings as required.
  • To contribute to daily handovers between shifts, to ensure effective communication between staff, including all communication documentation.
  • To act as a shift leader as required at the location.
  • To support and administer medication following relevant training in line with the support plan and individual guidelines.
  • To support and facilitate financial transactions for service users and the Trust in line with the individuals support plan and the company policy and procedures.
  • To support service users health and wellbeing, to include attending appointments, hospital admissions and promoting health styles.
  • To work in accordance with rota system, including working unsociable hours at weekends and some bank holidays. Sleep-in duties may be required at specific locations.
  • To undertake, any reasonable travelling requirements, in order to fulfil the tasks and responsibilities of the role.
  • To include driving Brainkinds and service user vehicles, ensuring service user activities/appointments are fulfilled where an appropriate driving licence is held.
  • To support service users, if requested, on holidays away from the location.
  • To undertake any other duties as may from time to time be specified by the Trust that is within the level and responsibility appropriate to the grade of post.

PERSONAL DEVELOPMENT, SUPERVISION AND TRAINING
  • To ensure that all essential and core training is undertaken and remains current, in line with requirements of the role.

  • Participate in regular support and supervision from line management.
  • In accordance with relevant guidance and training, to support service user(s) who may present challenging behaviour and complex needs, implementing approved strategies in accordance with the relevant programmes. To ensure that knowledge and understanding is updated and current.
  • To be aware of all service user support plans and programme changes in order to assist self and colleagues to support services users as required.
  • Continuously develop professional knowledge and expertise, attending and participating in learning and development events, meetings, conferences, and workshops (as requested/approved by line management) to ensure up to date knowledge relevant to the role.
  • Attend and participate in regular team and other meetings, both internal and external.
  • May be required to carry out training in addition to this substantive post.
