Job summaryThe Transitional Care Flow Discharge Co-ordinator is an exciting role within the transitional care and patient flow.
To work as LCHS representative working in conjunction with band 6 Clinical flow Practitioner in the Transfer of Care Hub's (ToCH) in Boston and/or Lincoln acute hospital sites.
The successful candidate will be working with the ToCH as an advocate for our service and dealing with stakeholders on a daily basis. They must be able to work with daily challenges that reflects LCHS while working for the best outcome for the patient.
The successful candidate will discuss optimised patients as they are declared on hospital ward communication system, with the ability to escalate to the Transfer of care practitioner both remotely and face to face to ensure smooth flow from the acute to Transitional care and flow.
Our Transfer care Team sits within the Transfer of Care hubs and within the community working as an MDT. Catering for patients coming out of the acute hospitals, both in and out of the county. Transitional Care is a function of the Integrated Community Teams and is designed to promote care for patients needing further therapy post discharge, prevent unnecessary acute hospital admissions and premature admission to long term residential care, support timely discharge from hospital and maximise independent living. We pride ourselves on using the Home First principle for each patient and ensuring the patient gets the correct care and the correct venue.

FULL TIME TO COVER A 7 DAY WEEK SERVICE
BASE- Pilgrim hospital, Boston and Lincoln County Hospital
We have an exciting opportunity within our Transitional Care and Flow Discharge team for someone to join us to support within the Transitional of care hub.
The post is face to face role that supports our patients in their care journey both to be able to actively assist the Acute hospitals in the flow of the patients discharge and our service within Transitional care. The role also involves the interaction and working alongside but the public and professionals and also can involve compiling reports and wider system calls.
We require someone that has good communication skills and keyboard skills and is familiar with system1.
You will need to show that you are a team player supporting your service and willing to be adaptable as well as supporting the wider acute TRUSTs. You will also be able to work independently and within a timeframe to ensure that pathway processes are followed without delay to our patients needing input from our hospitals, home and care settings.
