ESNEFT are paving the way with their huge advances within Integrated Community Services (INT's). Come and join one of the Integrated Neighbourhood Teams within Ipswich and East and play your part in keeping patients safe and well within the comforts of their own homes.
An exciting opportunity has arisen within the Integrated Neighbourhood Teams covering Ipswich West & Ipswich East.
We are looking to recruit Community Nurses on a permanent basis for a full time position, part-time hours would also be considered.
These are diverse caseloads which are lead by a band 7 clinical lead and three full-time band 6 senior nurses.
We are fortunate to be co-located with our other community nursing, therapy and social care colleagues as well as our out of hours nursing and therapy colleagues. We also enjoy the benefits of having the post graduate education team and legacy nurse to support in achieving the community specific clinical skills.
The service is delivered between 8am and 6pm 7 days a week.
Many of the 8 INT teams are co-located with our social care colleagues and wider MDT providing huge benefits to patient outcomes. An improved INT structure has allowed for huge advances in not just delivering frontline care but a concentration on making longer term health and social care plans through local data and the development of local delivery plans.
