An exciting opportunity has arisen for a registered nurse to join our dynamic integrated respiratory nurse team, delivering a high quality service for patients within Monmouthshire locality.

Candidates must be enthusiastic, self-motivated and be committed to delivering and developing our well-established community service.

The role involves undertaking treatment optimisation, clinical oxygen assessments and reviews in both the patient's home and the clinic setting.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

The post holder is responsible for the delivery of nursing practice across primary and community care populations working within the Primary Care. The focus of the role is both the delivery of evidence based practice for patients presenting with a respiratory condition, and the provision of preventative health care.

The post holder will provide specialist nursing knowledge support to other health care professionals and provide advice for patients/carer and family with Long term respiratory conditions and those requiring oxygen therapy.

As an autonomous practitioner the post holder is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients.

In addition, they will work collaboratively within the Primary and Community Healthcare Team and other respiratory nurse specialists to meet the needs of all patients, support the delivery of policy and procedures, and provide nurse leadership as required.

Responsible for the assessment, development, implementation and evaluation of patient needs. The post holder will assist in the implementation of Health care policies and will promote evidence based practice and act as an accountable role model. They will work clinically alongside members of the Nursing Team and wider multi-disciplinary teams to ensure the provision of effective, efficient, appropriate quality evidence based care to patients.
