Job summaryWe have a great opportunity for a Senior Clinical Assessor to join our award-winning End of Life Hub Care Hub, known as ECHO. This is a unique collaboration with partners across Coastal West Sussex, including our two Local Hospices, the Acute Trust and a specialist Macmillan Team.The ECHO Hub operates 365 days per year, 7 days a week, 24 hours a day, including Bank Holidays & weekends with shift patterns ranging from 06:45-14:45 & 11:15-19:15.
Main duties of the jobThe aim of this service is to provide a responsive, dynamic, flexible partnership-based approach for patients registered to the service.We are looking for people who have experience in community care, frailty or end of life care. Excellent clinical and communication skills are required, and a want to help make a difference to patients who are in their final stages of life. Palliative care experience would be an advantage.You will be passionate about end of life care and have a good knowledge of frailty, with the ability to learn and develop as needed. Excellent organisational and communication skills are essential.
