We have an exciting opportunity for a motivated, forward thinking and dynamic individual to be part our community based Proactive Care Team. The role of the Therapy assistant is to support the Physiotherapy & Occupational Therapy team in delivering their interventions. The post holder will also support the wider multidisciplinary teams with more generic tasks when required.

This is a rare opportunity for the post holder to work in a large and supportive MDT in the delivery of a community based integrated model of healthcare for individuals with the most complex health needs. The team see people in their own homes to deliver preventative and rehabilitation focused interventions.

To deliver a high standard of health care supporting both Occupational therapists and Physiotherapists, together with the wider MDT, to assess, plan and implement a range of therapeutic interventions to improve the physical, cognitive and functional ability, mood and well-being of patients.

The right candidate will have experience in working in health and/or social care settings who had an awareness of Occupational Therapy and Physiotherapy. The post holder will have excellent communication skills and can work effectively as part of a MDT.

To manage a delegated caseload delivering physiotherapy and occupational therapy independently treatment goals independently and with guidance evaluating and adapting interventions.

There will be additional generic duties as directed by the wider MDT.

To facilitate safe and timely interventions.

To manage and prioritise own caseload delegated by qualified therapists / MDT.

To contribute to team and service development.

To contribute to training and support to other team members and students.
