Salary Range: Band 3 - £22,717 - £24,478 per annum
Hours: 22.5 per week, Weds - Friday

St Gemma’s Hospice is seeking a Palliative Care Support Worker who is looking for an opportunity to work within a small but supportive Outpatient team, working closely with the Therapy team to support the re-enablement and encourage self-management skills with our palliative care patients and their families. The role provides opportunities for close multi-displinary working with therapy colleagues as well as medical, nursing, family support and spiritual care teams, as well as experience in inpatient, outpatient and Community settings.
Role will include facilitating outpatient groups, holding a telephone support call caseload, and working out into the community fitting equipment, carrying out mobility and exercise practice. Experience in an HCA or Therapy Assistance role previously is required.
