Job Summary
You will work as part of a multidisciplinary team to deliverenhanced clinical care. As an autonomous practitioner, you will be responsiblefor the care delivered, and will need to demonstrate critical thinking andclinical decision-making skills. Youwill provide care for patients from initial history taking, clinicalassessment, diagnosis and treatment. Asan Enhanced Practice Nurse, you will also contribute to and attend educationand training to other nurses, multidisciplinary team members and students. Youwill help us to transform and modernise pathways of care, enabling the safe andeffective sharing of skills across professional boundaries. We also encourageprofessional development and can offer internal and external education to helpyou explore your specialist interest.
Primary duties and areas of responsibility
Works as part of a MDT within the PCN.
Consults with patients, their family, and themulti-professional team to undertake assessments of patient need and devise andevaluate complex care plans.
Evaluates and analyses clinical problems usingtheir clinical knowledge, seeking out and applying relevant evidence, enhancedtechniques, interventions, and equipment to make clinical decisions.
Delivers enhanced clinical care in the context ofcontinual change, challenging environments, different models of care delivery,innovation and rapidly evolving
technologies using analysis and their underpinningknowledge to manage complex interventions.
Teaches and advises patients and their families onhow to manage their condition or support the multi-disciplinary team to do so.
Participates in clinical audits and researchprojects and implements changes as required, including the development, andupdating of practice protocols / guidelines and procedures locally.
Works within national and local protocols wherethese exist.
Recognises boundaries of their practice and knowwhen and to whom patients should be referred.
Using the principles of delegation, delegates workto other members of the MDT and take responsibility for the delegated activityas appropriate.
Demonstrates initiative and are creative in findingsolutions to problems.
Provides supervision to trainee nurse associates,nurse associates and general practice nurses.
Facilitates and supports the development anddelivery of innovative training and education to staff, to improve competenceand confidence, in line with best practice and guidance related to area ofexpertise.
Participates in and/or lead formal training eventspromoting best practice in area of expertise.
Provides professional and clinical leadership andmentorship, acting as a clinical role model on the delivery of evidence-basedpractice.
Advises and supports on the development anddelivery of work streams relating to area of expertise as part of the PCNsstrategy, where appropriate.
Works proactively with key local and national stakeholdersto develop more integrated care pathways and holistic models of care, throughdissemination of specialist skills and knowledge.
Contributes to the delivery of local policy,procedures and protocols for the PCN to improve identification, assessment,ongoing support for patients and their family carers.
