Job summary

The post holder will support the GPs in coordinating all keyactivity including improving access to services, providing advice, andinformation, and ensuring health and care planning is timely, efficient, andpatient-centred. The role will include supporting digital initiatives andincludes responsibilities for the co-ordination of the patients journeythrough primary care.

Main duties of the job

Responsible for case summarisation and discussion of patients

Visit patients incommunity, home or care home settings to assess and discuss their care needsinvolving carers as appropriate. Video links maybe required to aid visual assessment of the patient.

Ensure regular and consistentcommunication with care homes regarding patient progress and any complications

Measuring and recording following physiological measurements in routinepresentations:

Blood pressure

Pulse rate and rhythm

Temperature

Oxygen Saturation

Respiratory Rate
