In 2020 the service was highly commended by the NICE Shared Learning Awards for ‘Implementing Clinical Support for Informal Carers’, and in 2The Carers Health Team won the NHS Parliamentary Care and Compassion Award for the South East.
We provide professional health care input for informal adult carers across West Sussex.
The role involves lone working, home visiting, GP clinics, health promotion and outreach work (which can sometimes be either at the weekend or in the evening) including giving presentations and attending multidisciplinary meetings.
This role involves caseload management and integrated partnership working with the voluntary sector.
The purpose of this service is to support carers through health and wellbeing interventions so that the carer and cared-for person achieve the best possible quality of life; relief from symptoms, and flexible choices about their health management. This is an anticipatory, health promoting model of care for adult carers and their cared-for and includes the identification of new carers.
In order to meet the stated outcomes you will be undertaking:
- An holistic and clinical assessment of health, social and environmental needs of carers living in the community and carry a caseload of carers (and sometimes their cared-for) to enable proactive choices and advanced care planning with regard to the management of the condition and caring situation.
