Job summary

The primary focus of therole is to work as a Nurse Practitioner providing high quality and timelypatient centred care. You will work as apart of a team, within a defined area of practice and with access to supportfrom a GP.

Main duties of the job

Actas a clinical practitioner, demonstrating clinical competence and a knowledgebase beyond those associated with conventional nursing roles

Undertakeconsultations with a range of patients as an autonomous practitioner taking ahistory, carrying out physical examination, using knowledge and clinicaljudgement to diagnose

Makefinal diagnosis, decide on and carry out treatment, including theadministration and/or supply of medicines working to address patients needs

Useknowledge and experience plan and provide skilled and competent care to meetthe patients health and social care needs

Makedirect referrals to primary care GP, secondary specialist services within locallyagreed pathways, guidance and protocols

Identifyand take appropriate actions to address risks to the patient, this willinclude;

Identificationof emergency care needs requiring despatch of a 999 ambulance

Safeguardingchildren and protection of vulnerable adults issues which require referral onto other agencies in line with local policies
