We are looking for a suitably experienced, motivated and enthusiastic Physiotherapy HCSW to work as part of the rehabilitation team in the Deeside area. The post holder will be expected to undertake specific skilled rehabilitative support work to aid implementation and progression of physiotherapy led interventions for patients. This may involve working in a variety of settings including, community hospital, patients own home or other appropriate venue. There will be an expectation to work both alongside the physiotherapy staff and independently/unsupervised at times. Multi-disciplinary and multi-agency working is core to all interventions in our patient centred approach and this role will involve working closely with these extended team members.

The patient caseload will be overseen and supported by a Physiotherapist. The successful candidate will undertake treatment application with both progression and regression of activity, help motivate individuals and encourage self-management through activity, exercise and health promotion. Support will be available from within the physiotherapy team alongside structured supervision for caseload management including discussion around opportunities to enhance skills in line with KSF and agreed personal development plans.

Ongoing professional development is supported through local in-service programmes and NHSG HCSW programmes.

Completing an in-house training package to ensure competency is a requirement of the role.

The physiotherapy team support clinical placements of physiotherapy students, alongside other health related students, which includes students spending time with a HCSW.

The pattern of the hour distribution across the week is negotiable but will be led by the service need.

A full UK driving licence and access to a car is essential for this post.
