Main duties of the jobThe Home Care Worker will work without direct supervision in the home of the service user providing care and support including personal, social and domestic support whilst adhering to the service user's individual care and support plan.
You will work closely with Health and Social Care colleagues to keep people safe and well to avoid unnecessary admission to hospital, or to support people to return home and live independently at home.
Primarily you will work with people in their own homes and communities, with training and shadowing opportunities available in the hospital.
