Are you an experienced Healthcare Support Worker looking for a new challenge in a community setting? Come and join our Community Care Teams and help us to continue providing high quality and innovative care to the people of Harrogate and Rural area, in the comfort of their own homes.

As a Community Healthcare Support Worker with Harrogate District Foundation Trust, you will be based in one of our 4 Community Care Teams. The service runs between 7am and 8pm every day of the year. We offer flexible shift patterns.

To assist in assessing people's health and social related needs. This will include:

  • Assisting in the assessing of peoples’ health and social care needs
  • Taking and interpreting physical measurements and samples from named patients as delegated by the team
  • Monitor changes in patients, correctly interpreting their meaning and informing the team when changes might be significant
  • To assist in planning, delivering and evaluating nursing care and rehabilitation programmes to address peoples’ health and social needs according to their care plans.
