We have exciting opportunities for a Band 5 enthusiastic, motivated, and keen nurse to join our established multidisciplinary Care Community Team at Chester South, to support and deliver a range of quality nursing services to our patients.

There is one 9 month temporary post available, Part time - 22.5 hours per week.

Our eight care community teams are based in various locations within Chester, Ellesmere Port, Neston, Frodsham and the Rural locality covering the geographic area of West Cheshire. It’s important that you can drive and have access to your own vehicle for work. But, of course, mileage expenses are reimbursed in addition to your hourly rate.

As a Community Staff Nurse the successful candidates will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach.

You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care – including the provision of three GP surgeries.
