As a Keech Senior Adults Nurse you will prioritise excellence in care while we nurture your growth in the field of palliative care.

To work as a member of the Hospice multi-disciplinary care team to work towards the achievement of the aims and objectives laid down in the philosophy of Keech Hospice Care.

To ensure each patient and family receives a multi-professional assessment, their needs identified, and a plan of care implemented and evaluated in collaboration with other agencies and services that may be involved.

To work flexibly across Adult Care Services as required to ensure continuity of patient care (Adult inpatient unit, Wellbeing Centre, Community Services, Out of Hours service).

Your experience of caring for patients with a palliative diagnosis, perhaps even drawing from your own personal experiences, equips you with transferable and invaluable skills for this role.

  • Supervise and participate in the delivery of holistic patient centred care
  • Manage and prioritise own workload, ensuring continuity of service and crisis intervention.
  • Establish effective communication processes with patients, families, colleagues, and the broader multi-disciplinary teams and external agencies.
  • Undertake patient assessments in the hospice, their homes or place of care.
  • Support and coach staff to provide high-quality palliative and end-of-life care, including pre bereavement support.
  • Provide professional and managerial leadership, supporting colleagues as and when required.
  • Take a lead role in planning discharges from the hospice, ensuring a smooth transition of patient and family care and support.
