Anexciting opportunity has arisen for Central Manchester Networks ltd to recruit a PCN Dietitianto base within West Central Manchester PCN. WCM PCN is a group of GP practicesworking across Central Manchester. Primary care networks think about the widerhealth of their population, taking a proactive approach to managing populationhealth. By better understanding their wider communities, Primary Care Networksare able to focus on preventative care by tackling health inequalities andidentifying those who would benefit from targeted, proactive support.

Main duties of the job

Thepurpose of this flexible and exciting role is to deliver a bespokecommunity-based dietetic programme, which best meets the needs of WCM PCNservice-users. This role is one of an enthusiastic, creative and innovativepractitioner who works autonomously, but also involves liaison with otherprofessionals, both in primary care and relevant community services.

Deliver a programmeof dietetic support which meets the unique and diverse needs of service-usersfrom across the Primary Care Network. The current programme running Real FoodReal Health encompasses a person centred, strength based approach to helpingthe community to understand about their diet and how this impacts their health.It is currently being delivered by a health coach and a dietitian from aneighbouring PCN and producing excellent results.

Deliverservices in a variety of settings, including but not limited to: communityclinics, GP surgeries, patients own-homes, community venues, and groupsettings.

Promotethe role of the dietitian within the GP surgeries and wider network byattending meetings and events to network with other community workers.

PCN Resource and Advocacy:

Act as a PCN, departmental and organisational resource of expertise in dietetic/nutritional advice.

Support the GP practices to refer patients to appropriate dietetic services across Manchester.

Develop and sustain excellent working relationships with the practice teams within the PCN, working in a way that is collaborative and informed.

Direct Clinical Work:

Utilising specialist dietetic skills and knowledge to assess, plan, implement treatment and delivering dietetic interventions.

Use specialist knowledge and critical appraisal skills to make clinical judgments in complex case management and formulates dietetic care based on best practice.

Support the disease preventative agenda of promoting healthy eating and preventing risk factors for ill health.

Service Development:

Plan, adapting and develop the service to deliver interventions which best meet the needs of the cohort you are supporting.

Work with community services and assets to deliver a strengths-based, community-led approach.

Maintain thorough and secure patient records. Collating data and evidence to support research and evaluation of the service.

Training and Staff Support:

Comply with the national and local professional Standards of Practice and Codes of Conduct relating to Dietetics.

Working with other team members to develop skills and support wider service delivery. You will be expected to contribute to wider Shared Health activities and discussions through involvement in team meetings.

Support the development of junior members of the team through education and advice.

Contribute to the delivery of clinical supervision, training and mentoring of wider dietetic team, students and members of the wider MDT.
