We at RAVEN Primary Care Network are looking for a motivated and enthusiastic person to join our newly established team of Care Co-ordinators supporting our Primary Care Network of five practices in Rotherham.

The role is advertised as a full time position however the Primary Care Network is also open to candidates looking for a part time role.

The successful candidate will be supporting the practices in co-ordinating care for patients in areas such as cancer diagnosis, learning disabilities and assisting practices in supporting patients in their aligned care homes. You will join a team of three Care Co-ordinators who support and work alongside a wider range of staff working across all practices within the PCN.

The role of the Care Co-ordinator is a new and growing role within primary care and we hope for a candidate who can grow and adapt with this position.
