Xyla Health and Wellbeing, part of Acacium Group are looking for Health Coaches who will be delivering individual patient assessments for the National Diabetes Prevention Program. You will be supporting patients to help them overcome health barriers and facilitating health improvements.

Every day you will…


  • Conduct Individual Assessments with service users over the telephone, which includes validated questionnaires and motivational interviewing
  • Handle sensitive data with attention to detail, and maintaining accurate service user records
  • Confirm details of patient group sessions and providing information on the programme when required
  • Provide signposting opportunities to support services users to access other wellbeing services
  • Support the service user engagement strategy to ensure adherence to the programme and minimise drop-outs
  • Provide support to the Patient Support Team when required

To thrive in this role, you must have


  • Undergrad Degree in Nutrition or Health Related Science, e.g Sports, Exercise Science, Psychology, Health Psychology or equivalent OR a Fitness Qualification (REPS Level 2 and/or 3), with experience of delivering to groups of adults
  • Excellent customer service and interpersonal skills
  • An empathetic nature, with a willingness to want to help and support people
