York and Scarborough Teaching Hospitals NHS Foundation Trust is looking to recruit an enthusiastic and self motivated Discharge Liaison Officer.

You will be part of the Patient Flow team, working across the acute medical, surgical or elderly wards to optimise discharges and support patients and staff to negotiate discharge plans. The Discharge Liaison Officer works closely with; ward staff, Social Care, Continuing Health Care and the voluntary sector.

Therefore you must be able to demonstrate collaborative working, as well as being able to build effective relationships with colleagues from other healthcare professions.

This role entails working closely with a wide multidisciplinary team, to facilitate the timely discharges of patients from the hospital environment.

  • Excellent communication skills
  • Organised and able to prioritise workload
  • Perform well under pressure in a supportive team

You will benefit from working within a small, friendly, experienced, pro-active team who are looking for a colleague with a positive approach, and has a flexible and patient centred approach.


Please note you will be expected to join a weekend rota for this post.


The ability to escalate concerns via appropriate channels of escalation, is also essential in this role. The successful candidate will be required to use a variety of computer applications, and should be confident in IT skills.
