We have a vacancy within our new Pathway Hub for a Patient Care Coordinator.

You will play a key role in delivering our patient experience strategy, ensuring our customers have a first-class experience and receive the care and support they need.

Full time: 35 hours per week, Monday to Friday


Working Pattern
: Shift pattern 8am to 6pm

As a Patient Care Coordinator, you will:


  • Facilitate the delivery of outstanding care by being responsible for the co-ordination of administrative tasks and processes, ensuring these are managed in accordance with targets and agreed timescales.
  • Be responsible for providing a high standard of professional and patient focused services across a range of specialities and modalities.
  • Ensure patient records are maintained and that all correspondence, reports and results are uploaded correctly and in a timely manner. 
  • Track patients in accordance with RTT and DMguidance and standards. 


The ideal candidate will have:


  • Excellent empathy, communication skills and exceptional attention to detail.
  • Prior experience in a customer service role, ideally within a healthcare setting.
  • Knowledge and understanding of RTT and DMreporting requirements.
  • Ability to remain calm and professional in a very fast-paced, constantly evolving environment.
  • Strong organisational skills and attention to detail.
  • An ability to cope with the sensitive nature of work, and to deal with challenging and difficult behaviour in an appropriate manner.
