Job summaryAn exciting opportunity has arisen to become a senior enhanced nurse specialist as part of a heart failure specialist team. We are a close-knit team supported by senior consultants and other members of our MDT such as pharmacist and palliative care.
We provide care in both inpatient and outpatient settings and are involved from first diagnosis to end stage heart failure. The role will be focused on ensuring evidence-based medicines are optimised, providing lifestyle and self-care advice to patients and carers, and ensuring the patient has a comprehensive management plan. We hold weekly multidisciplinary meetings where we can discuss complex patients and access the expertiseof our heart failure and cardiac rhythm management consultant colleagues as well as advanced heart failure specialists and palliative care experts.
We are excited to be part of new and inventive ways of working through using information technology and services such as virtual wards to support patient care. The post holder will be a key member of the teaminfluencing service development and involved in internal audit, research, and inputting data into the national heart failure database (NICOR).The post holder will be offered many opportunities to continue their professional development. They will have the chance to grow skills in enhanced practice as an autonomous practitioner.
Main duties of the jobAdditionally, there will be the possibility to progress your leadership skills through training programmes within the trust. You will receive a comprehensive induction programme, will be supported by senior nurses, and follow the BSH competency framework to guidedevelopment in your role.
