About The Role
We are seeking to recruit permanent part time staff, who are enthusiastic and self-motivated people, to join our team to undertake the role of CRS Rehab Assistants for the Community Recovery Service.
The Hounslow Community Recovery Service marks an exciting step towards health and social care working together. The Community Recovery Service is an integrated service with Hounslow and NHS Trust and aims to support people's independence following an injury, illness or a change in life circumstances and living at home. The team provides a period of rehabilitation and reablement to help local people in Hounslow recover and regain their independence. It brings together different health and social care professionals into a single integrated team.
You will need to work flexibly, to support vulnerable residents, their careers and staff in order to meet the needs of the service cover a period seven-day service 8.00 a.m. to 22.00 p.m.
About The Team You’ll Be Working In
You will be part of a large supporting multidisciplinary team which includes, Therapists, social workers, and nurses.
- Carry out personal care support as required for clients with rehabilitation goals to maximise independence, where the client’s current ability requires another to complete part of / all the task on their behalf. This includes, but is not limited to, washing, dressing, shaving, managing continence, toileting, bathing, transfers in / out bed, administering medication. Training will be available.
- Work with clients referred to Community Recovery Service as required. To see clients in their home, or other community settings, and when safe and appropriate, to work with or without direct supervision in Community settings, carrying out relevant risk assessments and following policy guidelines on lone working.
- Demonstrate a basic understanding of the effects of disability, with specific regard to community rehabilitation, during client intervention.
- Carry out and contribute to client assessment as delegated by the occupational therapist or physiotherapist and provide feedback on clients' occupational performance.
You will be given the opportunity to be creative in this role, developing strategies and practices improving ways of working.
- You will receive significant support from the CRS Assessors, CRS Co-ordinators and the Registered Manager.

- Understanding of concept of enabling clients through the therapy process.
- Able to carry out moderate to intense physical effort throughout the working day and carry out concurrent activities.

NVQ 2, Care Certificate or Equivalent Experience.
