Job summarySt Davids Clinic have a fantastic opportunity for a Clinical Pharmacist to join our well-established team.
The postholder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face and remote structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manager repeat prescriptions authorisations, acute prescription requests, while addressing both the public health and social care of patients in the GP practice.

- Provide Structured Medication Reviews
- Medication reauthorisation
- Processing hospital discharge/clinical letters
- Actioning MRHA alerts
- Community pharmacy queries
- Contribute to the improvement of health outcomes in long term conditions as part of the multidisciplinary Primary Care Team.
- Other general tasks as required.
About usSt David's Clinic is a supportive, forward-thinking teaching/training practice involved in research, with a practice list size of 20,700 and a friendly multi-disciplinary team of other dedicated and highly skilled healthcare professionals comprising of 6 partners, 7 salaried GP's, 8 registrars,with an ACP who along with our Physician Associate deal with home visits and manage fortnightly MDT meetings working collaboratively within other like minded clusters, sharing ideas and resources. 1 Physiotherapist, 2 pharmacist and a pharmacist technician providing support to help with repeat prescriptions/medication reviews, 4 Practice Nurses, 4 HCA's providing care for patients and their families in a way we believe is best, delivering high service standards, sharing best practice and actively improving the way we work. Situated in the centre of Newport with an easy commute from Cardiff.
