About The Role


The Vale of Glamorgan Community Mental Health Service for Older People is seeking an experienced and dedicated Qualified Social Worker to join our multi-disciplinary team. Our team comprises mental health professionals, including social workers, psychiatric nurses, and psychiatrists, who specialize in providing secondary mental health care for adults living with dementia and individuals over 65 years with mental health problems.


Key Responsibilities


Undertake assessments of citizens in accordance with statutory duties under relevant legislation.

Plan and commission services to support adults with care and support needs, as well as carers, to achieve personal outcomes through co-production.

Work in partnership with citizens, carers, and adults at risk to provide opportunities for fulfilled lives in a way that matters to them.

Negotiate and coordinate applications for resources and collaborate with internal and external partners, both statutory and voluntary.

Manage a specific caseload, prioritizing workload demands appropriately.

Maintain up-to-date electronic case files and relevant documents, ensuring accurate performance management records and statistical data.

Prepare for and participate in supervision and Personal Development Review sessions with your supervisor/line manager.

Identify personal development and training needs to maintain Registration with Social Care Wales.

Undertake training for continuous professional development, including training to become an Approved Mental Health Practitioner or other senior roles.

Supervise or mentor other staff or students, contributing to training programs and Team Practice Issues meetings.

Participate in office duty rotas and other activities associated with a multi-disciplinary team.

Ensure compliance with the Council's Financial Regulations, Standing Orders, Policies, and Procedures.

Implement the Council's Equal Opportunity Policy and adhere to Health and Safety legislation and relevant Council policies.

Report any safeguarding concerns to the appropriate safeguarding officer without delay.

Carry out other duties and responsibilities commensurate with the grade as reasonably required by the Chief Officer.


Qualifications And Experience


Degree in Social Work and registration with Social Care Wales.

Proven experience in a similar role, preferably within a mental health or older people's services setting.

Strong assessment and care planning skills.

Excellent communication and interpersonal skills.

Ability to work independently and as part of a multidisciplinary team.
