Kingswinford & Wordsley Primary Care Network is a network of 5 practices with a list size of over 50,000 patients. This role exists to support the delivery of the Kingswinford & Wordsley Primary Care Network agenda. The post will form an integral part of the PCN leadership team working alongside the Practice Managers and the Clinical Director and the PCN board members.
Main duties of the jobResponsibilities
1) Working with the senior management team to develop and improve specific PCN services in line with the PCN DES
2) Assist with the management and induction process of other ARRS staff where required
3) Design rotas and a structure around ARRS staff within the PCNs
4) Work closely with the PCN Clinical Director to improve patient services
5) Identify a persons care and support needs, and explore their options to meet
these into a single personalised care in collaboration with the patients primary care team
6) Assist in providing coordination and navigation for elderly people and their carers across health and care services, alongside working closely with social prescribing link workers, occupational therapists, dieticians
