A proficient health care professional who provides expert evidence based practice and clinical expertise in a specialist area (Colorectal). An experienced practitioner with decision- making autonomy, responsibility and accountability within agreed guidelines and protocols, who contributes to the leadership, development and supervision of others. The practitioner works collaboratively with others or independently to meet the needs of the patients and their families. It is expected they will also participate in the education and development of other staff members and assist with the development of clinical services and enhanced practice across the Trust.
- Responsible for leading and delivering the triage of all suspected colorectal cancer referrals.
- Responsible for the ordering of appropriate investigations per-protocol for ‘Straight to Test’ (STT) patients.
- Responsible for first outpatient telephone triage for patients not going to STT
- Responsible for the review and interpretation of diagnostic results/reports.
- Responsible for enacting an agreed diagnostic grading protocol and implementing protocolised pathway outcomes (e.g. discharge with template letter).
- Management of the Cancer Support Worker, including supervision of communication with patients and other MDT personnel (e.g. preparing patients for their procedures, collating information on the standard of service delivery, communicating ‘next steps’ and ‘no-cancer’ diagnoses).
- Ensuring timely documentation on Somerset Cancer Registry
- Support the delivery of first outpatient appointments for patients who are not triaged as suitable for STT. Tandem delivery of consultation (with HCP), progressing under supervision towards independent delivery under supervision.
